Halifax Regional Medical Center, the leading healthcare provider in the Roanoke Valley, provides critical outpatient case management services to community residents suffering chronic disease such as heart failure. These services are offered in order that patients can avoid costly readmissions to the hospital and traumatic visits to the hospital Emergency Care Center. Outpatient case management services to heart failure patients include follow-up telephone calls once they are discharged from the hospital. Case management personnel, who are licensed nurses, make these follow-up calls to remind patients to take their prescribd medications and monitor their diet and exercise regimens as prescribed by their physicians. Additionally, outpatient case managment personnel counsel family members to help them become better informed about their loved ones illness. The Outpatient Case Management Program is funded in part by the Roanoke Valley United Way. These funds enable Halfiax Regional to meet the needs of these chronic disease patients in the home setting, thus providing them the tools to maintain a normal daily lifestyle without constant visits to the hospital.
Halifax Regional is licensed for 204 beds, including psychiatric and nursery and has been operating in Roanoke Rapids since 1972. Halifax Regional is fully accredited by The Joint Commission. The hopsital offers an array of medical and surgical services, including 24-hour emergency care, backed by the latest technology. Halifax Regional strives to create a compassionate environment where its approximately 800 employees can deliver the best care every hour of the day.
